Privileges for medical workers in the Russian Federation
The privileges enjoyed by Russian doctors are set out in the fundamental document: Federal Law No. 323 of November 21, 2011 “On the fundamentals of protecting the health of citizens in the Russian Federation.”
According to this law, all medical workers have the right to receive:
- Decent working conditions in the workplace.
- Regular professional retraining and improvement of qualifications at the expense of the employer.
- Regular salary increases based on certification results.
- Professional activity risk insurance.

Shortened working hours and additional vacation
To attract doctors and nurses to rural areas, benefits are provided for rural health workers in the form of compensation for utility bills and a one-time payment for the purchase of housing. But they also have benefits common to all doctors in accordance with labor legislation.
Medical workers are provided with annual additional paid leave. Its duration depends on the specialty (Resolutions of the Government of the Russian Federation dated June 6, 2013 No. 482, dated December 30, 1998 No. 1588):

Article 350 of the Labor Code and Decree of the Government of the Russian Federation dated February 14, 2003 No. 101 established reduced working hours for health workers:

Results
In 2021, some health workers have the right to take additional time off in addition to their main vacation. So, if a specialist is a general practitioner (family doctor), then he is entitled to at least 3 additional days of vacation. Employees of medical institutions and organizations that provide mental health care, treat and fight the spread of tuberculosis and HIV infection are entitled to additional days of leave for the harmful conditions in which such specialists work. Moreover, for them the specific duration of leave depends on the position held.

Early retirement

They are assigned an early pension if the duration of their medical work in rural areas is at least 25 years, and the minimum IPC value in 2021 is 18.6 points. Payment of housing, utilities
In accordance with Decree of the Government of the Russian Federation dated December 30, 2014 No. 1607, monthly compensation for utilities is provided to health workers in rural areas working in federal government institutions in the amount of 1,200 rubles. In accordance with the procedure approved by Order of the Ministry of Health No. 121n, Ministry of Finance of Russia dated March 18, 2015 No. 39n, compensation is paid at the place of work along with wages.
As for medical workers of regional and municipal healthcare organizations, additional social support measures for them, including compensation for electricity payments, compensation for medical workers for gas in rural areas, are established by regulations at the appropriate level, and their volume varies in each subject. Depending on the conditions in that particular rural settlement, they receive a subsidy ranging from 50 to 100% of these expenses.
The subsidy also provides a benefit for rural health workers for electric heating based on the cost of 33 kW of electricity per month.
Within the limits of social norms, the following are subsidized:
- from 3 to 9 cubic meters of gas for heating 1 sq. m of housing area;
- costs for the purchase of 1.5 cubic meters. m of wood fuel or 2 tons of coal.
Procedure for receiving benefits
Legislative acts of the constituent entities of the Russian Federation provide benefits for health workers in housing and communal services in rural areas and also establish the procedure for receiving them. It contains a list of necessary documents and indicates where to apply for benefits. To receive them, you must contact the social protection department at your place of residence.
The standard list of documents for receiving utility benefits for health workers of regional and municipal medical institutions looks like this:
- statement;
- Russian passport;
- certificate of ownership of residential premises or a lease or rental agreement;
- certificate of family composition;
- a certificate or copy of the work book confirming work in a medical institution;
- SNILS;
- receipts for payment of housing and communal services confirming the absence of debts;
- checks for the purchase of solid fuel and its delivery (to compensate for expenses);
- bank account number for transferring funds.
Regulatory acts in each region establish how to apply for utility benefits for health workers in rural areas and specify the list of required documents and information.

What documents are needed?
The required package of mandatory papers must be specified at the place of residence in the social protection of the population.
The standard list of documents includes:
- a properly drawn up application;
- identification;
- diploma of education;
- employment history;
- extract from the place of employment for 3 or 6 months - depending on the type of benefit issued;
- certificate of registration or divorce.

- title documents for existing expensive property - if it is provided as collateral;
- passports of other family members (for children under 14 years of age - birth certificate).

Program "Zemsky Doctor"
The federal program “Zemsky Doctor” has established what benefits a health worker in rural areas has for purchasing housing.
Previously, medical workers under the age of 50 could participate in it. On November 2, 2021, Decree of the Government of the Russian Federation dated October 18, 2019 No. 1347 came into force, which abolished age restrictions.
One-time payments to health workers in rural settlements are provided in the following amounts:
- for a doctor - 1 million rubles;
- for a paramedic - 500 thousand rubles.
The conditions for the participation of health workers in the “Zemsky Doctor” program are established by Decree of the Government of the Russian Federation of December 26, 2017 No. 1640.
To receive payments to health workers in rural areas, specialists must meet the following requirements:
- higher or secondary medical education;
- moving to work in a rural area;
- availability of an employment contract with a medical institution for a period of at least 5 years.
To request payment, you must contact the regional health department and provide:
- statement;
- passport;
- diploma of higher or secondary education in the field of healthcare;
- SNILS and TIN certificate;
- a copy of the employment contract with the medical institution;
- The order of acceptance to work;
- Bank account details for transferring funds.
For those who have not yet purchased their own housing, regional laws provide not only compensation for utilities for health workers in rural areas, but also compensation for part of the payment for renting housing. Its availability and size must be checked with the social protection departments at your place of residence.
Benefits for honored doctors
The honorary title “Honored Doctor” (Decree of the President of the Russian Federation dated 09/07/2010 No. 1099) is awarded to practicing doctors for personal merit no earlier than 20 years from the beginning of their medical activities (doctors in X-ray laboratories and offices - no earlier than 15 years) and availability of awards from federal government bodies or government bodies of constituent entities of the Russian Federation.
Honored doctors of the Russian Federation have the right to become labor veterans when they reach retirement age and receive social privileges. They receive 50% compensation to health workers for utilities, free travel on public transport with a veteran’s ID, and other benefits depending on their region of residence.
