This is a paper that is given after registration at the place of residence of a child under fourteen years of age.
It is issued in one copy and must be attached to the birth certificate, since without it it has no legal force.
Form No. 8 on registration of a child at the place of residence must contain the following information:
- Full name of the minor;
- date of his birth;
- address of permanent residence along with postal code;
- the name of the official paper on the basis of which this certificate is drawn up and all the data contained in it, including details;
- Full name of the official who carried out the registration (with transcript), signature;
- name of the authority issuing the document;
- date of issue and stamp.
Certificate No. 8 can be given to a child only in several cases:
- at his birth.

- Moving to a new place of residence.
