WHAT TYPES OF CERTIFICATES CAN AN EMPLOYEE REQUIRE?

Employees may need different certificates. Vary depending on the purpose, content and place of provision. In this article we will look at examples of the most popular certificates.

The obligation to issue certificates to employees is established by the Labor Code of the Russian Federation.

Article 62. Issuance of work-related documents and their copies

3. Tax agents issue to individuals, upon their applications, certificates of income received by individuals and amounts of tax withheld in the form approved by the federal executive body authorized for control and supervision in the field of taxes and fees.

A certificate is issued based on the application. It is signed by the manager and accountant and certified with a seal.

Details of certificates are drawn up in accordance with the layout of document details given in Appendix A to GOST R 6.30-2003 “Unified documentation systems. Unified system of organizational and administrative documentation. Documentation requirements."

Why do you need a family composition document?

A document on family composition is required to be provided by various government agencies, as well as private organizations and even individuals. Government agencies request information about household composition in the following cases:

  • when applying for subsidies, compensation and other measures to help the poor;
  • when registering as those in need of housing or improved living conditions;
  • when registering maternity capital and compensation for preschool institutions;
  • when registering as participants in social assistance programs such as “Young Family” and other similar ones;
  • when taxpayers file tax deductions;
  • for military registration of men of military age.

The family composition form is requested:

  • courts: when considering alimony disputes;
  • divorces and determining the child’s place of residence;
  • in other litigation related to the acquisition and operation of housing;
  • banks:
      in the case of obtaining housing loans, mortgages, including on preferential terms for certain categories of citizens;
  • when transferring housing as collateral for other obligations;
  • when obtaining permission for transactions with mortgaged housing;
  • real estate agencies and individuals when completing transactions with housing (purchase and sale, rent, rent, etc.).
  • When making a request, it is important to take into account the specifics of the institution to which the information is submitted, since the validity period of the certificate also depends on it.

    FORMS OF CERTIFICATES ISSUED TO EMPLOYEES

    Certificate from place of work

    Such certificates occupy a special place among personnel documents. Their main purpose is to confirm certain facts, namely:

    • place of work;

    • position or profession (example 1);

    • work experience;

    • salary amount, etc.

    A certificate of employment is signed by the head of the organization or a personnel employee, if such a duty falls within his competence on the basis of an order on the distribution of duties.

    The salary certificate also has several varieties. For example, a salary certificate for the last three months of work (example 2) can be submitted to the social security authorities to recognize the family as low-income.

    Another common certificate - about the amount of wages for the two calendar years preceding the dismissal of the employee - is issued upon a written application from the insured person (his legal representative or proxy) no later than three working days from the date of receipt (registration) of the application by the policyholder.

    A dismissed employee needs this certificate to apply for benefits (temporary disability, maternity, child care).

    It is filled out either by hand with black or blue ink (ballpoint pen), or using technical means (computer or typewriter). Erasures and corrections are not permitted. The information contained in the certificate is filled out on the basis of accounting data and reporting of the policyholder. The completed certificate is certified by the organization’s seal (round), which is affixed in a specially designated place and should not cover the signature.

    Certificate for submission at the place of request

    Depending on the location of the request, the content of the certificate may vary. The contractor must check with the employee to which organization he will represent her.

    A certificate for presentation at the place of demand indicated by the employee is drawn up and registered in the same manner as certificates from the place of work - on the organization’s letterhead (example 3).

    What certificates are we talking about?

    A family composition certificate is a document illustrating how many people live in the same living space as the citizen receiving it.
    Previously, it was requested by social protection authorities when applying for benefits and other assistance measures. Currently, such documents are not required to be provided; government agencies themselves receive the necessary data through interdepartmental interaction. But sometimes such forms are required when making various transactions with housing (pledge, sale and purchase, donation) or in litigation. To avoid getting into a situation in which you are denied a service due to an inappropriate form or an expired document, you need to know that the family composition certificate has an expiration date. The period of its validity will be determined by the requirements of the institution that requests the data or the service received.

    Certificates for children's holidays: where and how to get the necessary documents

    All children's clinics of the capital's Department of Health operate offices for issuing certificates and referrals. There, parents can obtain the necessary medical documents for their child to relax in a children's health camp or in a sanatorium. On weekdays these offices are open from 08:00 to 20:00, and on Saturdays from 09:00 to 15:00.

    Parents can make an appointment for their child in such an office on the day of treatment at the clinic or in advance through the mos.ru portal .

    For a children's health camp you will need to issue three documents:

    — certificate in form 079/у (about the child’s health status);

    — certificate of no contact with infectious patients at the place of residence;

    — a certificate of no contact with infectious patients at the place of study.

    A health certificate can be obtained in a special office after the child has been examined by a doctor. , you do not need to make an appointment with your local pediatrician .

    A certificate of no contact with infectious patients is issued after the doctor has checked whether anyone from the child’s environment has suffered any infectious disease. The document can be obtained at your place of residence at the clinic and at the first-aid post of a school or kindergarten on the last day of your visit. A certificate of no contact with infectious patients at the place of study is required only if the child is sent to rest at camp during the first shift .

    These medical documents can be obtained from your local pediatrician, as well as from the office for issuing certificates and referrals. In addition, some camps and recreation centers may require other types of certificates. The list of documents must be clarified with the administration of the institution.

    If the child is vacationing in a sanatorium , then obtaining medical documents will take several days. It is necessary to issue a certificate in form No. 070/u (to receive a voucher to the sanatorium); a sanatorium-resort card and a certificate of contacts (issued at the place of residence, if the child attends school or kindergarten - at the place of study).

    A certificate in form No. 070/u is issued by a local pediatrician if the child has indications and there are no contraindications for sanatorium treatment. A sanatorium-resort card can be obtained both in the office for issuing certificates and directions, and from the local pediatrician. It is important to remember that it takes at least three days . The card is issued if the child has passed all examinations related to his illness. In some cases, additional tests may be prescribed.

    A certificate of no contact with infectious patients at the place of residence is obtained in the office for issuing certificates and referrals at the children's city clinic to which they are attached. The document is also done at the school or kindergarten first aid station. These certificates are issued no earlier than three days before the child’s departure and are valid for three days.

    Offices for issuing certificates and referrals appeared in the capital's children's clinics in 2021. They quickly receive the necessary documents without making an appointment with a pediatrician.

    Registration of documents in city medical institutions is becoming easier and more accessible for citizens. This is done online .     Who is authorized to issue the document

    • residents of an apartment or house;
    • owner of the premises;
    • heirs;
    • government agencies;
    • representatives of the above persons.

    Content requirements

    Confirmation of the registration of children, like any other citizen, is carried out through a special form called Form No. 9. It has a certain content.

    The law, represented by Order of the Federal Migration Service (FMS of Russia) dated September 11, 2012 N 288, regulated the content of form No. 9.

    It contains :

    • to whom it is issued, i.e. Full name of the minor resident, his year of birth;
    • type of registration, its date;
    • address;

    What is needed to obtain a birth certificate

    They won’t just give you a document to receive benefits. First, upon discharge after childbirth, one of the parents is given a certificate of birth of the child from the maternity hospital of the established form.     The driver's medical examination will not undergo major changes in 2022. It will continue to include the following specialists:

    Validity period of the certificate “Form 9”

    Legally, this document does not have a statute of limitations, that is, the law does not indicate how long it is valid. It is generally accepted that information from Form 9 is valid for 1 month from the date of issue. However, some companies that should submit the document independently set its validity period from 10 to 14 calendar days.
